Case Management Services – Coming July 2012!

The Pride Center of the Capital Region is proud to announce the expansion of our Center Support program!  Starting in July, for the first time we will offer Case Management Services to our community.

Lesbian, Gay, Bisexual, Transgender and Queer (LGBTQ) people often face barriers in accessing social services and receiving quality, culturally competent health care.  Our Case Management Services are designed to ease these barriers and also offers trans-specific health education information, social service coordination, assistance in obtaining identity documents, culturally competent medical and mental health referrals, and a variety of other health and human services.  Our Mental Health Specialist can also assist clients in navigating the health insurance system, entering or reentering the workforce and provide advocacy and support.

Our Case Management Services are the only trans-specific case management services in the Capital Region.

Case Management Services are free for youth ages 13 to 21 and operates on a low-cost, sliding-scale fee system for adults over the age of 21.  The first session is free and appointments are required to meet with the Mental Health Specialist; limited evening hours are also available.  To make an appointment for Case Management Services or ask for more information call 518.462.6138 or email casemanagement@capitalpridecenter.org.
